Thanks for the extra detail.  Everything looks sensible so far.

Can you check the contents of mdadm.conf on the initrd please.
e.g.

   cd /tmp
   zcat /boot/initrd | cpio -idv
   cat etc/mdadm/mdadm.conf

something like that.
