[PATCH 1/3] btrfs: move the remaining on-disk format to ondisk_format.h

There are still some on-disk structures in ctree.h: - btrfs_header - btrfs_root_backup - btrfs_super_block - btrfs_item - btrfs_leaf - btrfs_key_ptr - btrfs_node Move the definition of them to a dedicated header, and add comment explaining how btrfs ondisk schemas are split.
