In this case, # update-initramfs -k 2.6.32.43kerneltest -c should generate the requirered initrd, and # update-grub should make it known to grub. AFAIR, it is a known limitation/problem of make-kpkg that the initrd is not automatically built when you install a kernel compiled with that.
