The main change is just to maintain the glossary manually instead of by
the sort_glossary.pl script; includes a subsequent patch to demonstrate
the sort of cleanup that allows.

It don't see these in the latest maint or master, but I don't think the
following discussion turned up any objections.  Let me know if there's
something more you need from me.

You gave me exactly what I needed.  I simply forgot to pull
after the discussion petered out.
