PULL REQUEST - md updates for 2.6.38

Hi Linus,
 please pull some md update as below.

 There is an unfortunately high number for fixes for regressions - obviously
 someone hasn't been doing methodical testing as often as he would like to.

 Most of this is bug fixes.  For the few that are new functionality, most of
 them are related to our work to make md and dm able to share more code.
