You can try this. It's mainly for the 2.6 architecture and also it did not
discuss some of the fb functions (such xxxfb_cursor). But you should be
okay by combining the above and reading some of the drivers, notably vfb.c
vesafb.c, skeletonfb.c.

Note, this howto has an old version (for 2.2 kernels which also applies for 2.4)

I think you meant user apps for fbdev. I also haven't seen any docs for that.
You can look at the fbutils code for an example. Should be available on the
same site (linux-fbdev.sf.net)

Yes, initially anyway. But I might end up hacking fbdev if it doesn't do what
I need :)
Good suggestion (fbutils) I'll start there.
Where does that latest kernel stuff live? Is there a fbdev development tree
somewhere?
